Gift Certificate
Terms and Conditions

The gift certificate is subject to the following conditions, which may be revised from time to time:

1) The gift certificate is not valid until it has been paid for in full

2) The gift certificate can be revoked if the funds unused to pay for it are removed or reversed.

3) Gift Certificate may not be reassigned to any other person, and is only valid for the person named on the Gift Certificate

4) The person presenting the gift certificate must show proper picture identification - specifically a driver's license or passport, to redeem the gift certificate. Only Lauren Ashley Davis can approve the identification being presented.

5) Gift Certificate has no cash value and may not be used or redeemed anywhere, other than with Lauren Ashley Davis

6) A Gift Certificate may be revoked, with all monies fully refunded if Lauren Ashley Davis chooses not to provide services to the person presenting gift certificate.

7) Gift Certificate does not earn interest or appreciate in value overtime. It is only good for the face value of the gift certificate

8) Gift Certificates expire after 2 years from issue date

9) Refunds for Gift Certificates are subject to a $25 service fee, and may be refunded after the 2 year period, subject to proof of identity.

10) Refunds will only be issued to the named person on the Gift Certificate.

11) The Gift Certificate must be presented and permanently turned over to Lauren Ashley Davis to be used by the Gift Certificate Holder

12) If not all of the funds are used up by the services provided by Lauren Ashley Davis, the Gift Certificate holder is eligible for either a refund of the remaining balance, subject to a processing fee as outlined in the terms and conditions, or a new Gift Certificate shall be issued for the balance of the account, which will have a new 2 year term.
